Job summary

Team Select Home Care seeks a Home Health Aide (HHA) to provide skilled personal care and ADLs in patients' homes under RN supervision. You will perform assigned tasks, maintain infection control, and document visits for timely RN reporting.

Qualifications include a High School Diploma or GED, CPR/health certifications, and state eligibility. Competitive hourly pay, flexible scheduling, and ongoing training are offered.

Qualifications

• High School Diploma or GED required.
• 18 years of age or as state regulation.
• Maintains CPR certification and health certifications per state requirements.

Responsibilities

• Provide direct patient personal care and ADLs as ordered.
• Assist with simple procedures as an extension of nursing/therapy services.
• Maintain infection control practices and timely documentation.
• Complete visit records per agency policy and report changes to RN.
• Attend staff meetings and care conferences as scheduled.
• Follow agency policy and perform RN delegation tasks when applicable.
