Job summary

Team Select Home Care is seeking a Home Health Aide (HHA) to provide skilled personal care and ADLs to patients in their homes under RN supervision. You will report to the RN and collaborate with the care team.

Responsibilities include direct patient care, household tasks, and assistance with medications as allowed by state regulations. Strong documentation, confidentiality, and attendance are essential.

Qualifications

• Empathetic and compassionate attitude toward care of the sick.
• Ability to carry out directions, read and write.
• Effective communication and ability to treat patients with dignity and respect.

Responsibilities

• Provides services as ordered in the plan of care and per state law.
• Performs direct patient care, household tasks, meals, and other assigned activities.
• Assists with simple procedures as an extension of nursing/therapy.
• Assists in ambulation or exercises as assigned.
• Assists with self-administered medications as allowed by state regulations.
• Maintains infection control practices and adheres to agency policies.
