Job summary

Team Select Home Care is seeking a Home Health Aide (HHA) to provide skilled personal care and ADLs in patients' homes under RN supervision. You will report to the RN and follow the plan of care with strict infection control and timely documentation.

Requirements include a high school diploma or GED, being 18+, CPR/health certifications per state rules, and licensure as applicable. Competitive pay and benefits are offered.

Qualifications

• High school diploma or GED required.
• 18 years of age or as per state regulation.
• Maintains CPR certification and health certifications per state requirements.
• Maintains licensure or certification per state regulation.
• Ability to read, write and follow directions.

Responsibilities

• Provide direct patient care, meals, and household tasks as per plan of care.
• Assist with simple procedures as delegated.
• Support ambulation and prescribed exercises.
• Assist with medications ordinarily self-administered per state regulations.
• Maintain infection control practices and report changes to RN.
• Complete visit records timely and report to RN.
• Attend staff meetings and care conferences as scheduled.
• Maintain patient confidentiality and safeguard information.
• Participate in QAPI activities.
