Caringhand Home Health Solutions, Inc. is seeking a compassionate Physical Therapist to deliver in-home physical therapy services, assess patients, and develop individualized treatment plans.

This field-based, per-visit role involves travel to patients' homes throughout California, requiring organization, independence, and strong communication with physicians and families. The ideal candidate will have a CA PT license, 2+ years of relevant experience, CPR certification, and reliable

Qualifications

• Bachelor's or master's degree in physical therapy from an accredited program.
• Current California Physical Therapist license.
• At least two years of experience as a Physical Therapist.
• Experience in community-based or home health physical therapy.
• Current CPR certification (American Heart Association).
• Valid driver's license and reliable vehicle.

Responsibilities

• Provide PT services according to physician's plan of care and California requirements.
• Assess functional status, mobility, strength, gait, ROM and rehab potential.
• Develop and direct therapy programs; educate patients and caregivers.
• Coordinate with physicians and other healthcare professionals.
• Document progress summaries and assist with discharge planning.
• Supervise PT Assistants and advise home health aides as needed.
