Job summary

All Care and Comcare Home Health is seeking a compassionate Field Physical Therapist to deliver skilled therapy in patients' homes across our service area. You will assess, plan, and implement individualized treatment plans, educate patients and families, and coordinate with the interdisciplinary team to optimize outcomes.

Requirements include California PT license, CPR, driver's license, reliable vehicle, and willingness to travel to patient homes.

Qualifications

• Active California Physical Therapy license.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Willingness to travel to patient homes.

Responsibilities

• Provide comprehensive, skilled physical therapy services in patients' homes per physician orders.
• Perform SOC, ROC, Recert, and DC evaluations and documentation.
• Assess patient needs, plan interventions, monitor outcomes, and adjust care.
• Collaborate with the interdisciplinary team to set and evaluate goals.
• Educate patients, families, and caregivers on home exercise programs.
