Job summary

Caringhand Home Health Solutions, Inc. is seeking a compassionate Physical Therapist to provide in-home therapy across California. You will assess mobility, strength, gait, and ROM, then develop treatment plans and educate patients and caregivers to support recovery and independence.

This field-based, per diem role requires travel to patients’ homes. A valid CA PT license, CPR, and two years of PT experience are expected, with strong communication and organizational skills.

Qualifications

• Bachelor's or master's degree in physical therapy from an appropriately accredited program.
• Current California Physical Therapist license issued by the Physical Therapy Board of California.
• At least two years of appropriate professional experience as a Physical Therapist.
• Previous community-based or home health physical therapy experience.
• Current CPR certification by American Heart Association.
• Valid driver's license and access to a reliable vehicle in good working order.
• Strong verbal and written communication skills.
• Good organizational skills and the ability to manage patient care responsibilities effectively.
• Ability to work independently and provide therapy in patients’ home environments.

Responsibilities

• Provide physical therapy services according to the written physician’s plan of care and applicable California Physical Therapy Practice Act requirements.
• Assess and evaluate patients’ therapeutic, rehabilitative, and functional status, including home environment.
• Assess muscle strength, mobility, gait, ROM, and other functional abilities.
• Participate in developing the overall plan of care and determine patients’ priority PT needs.
• Initiate and direct appropriate physical therapy treatment programs.
• Instruct patients and families or caregivers in the proper use and care of therapeutic appliances.
• Educate patients, caregivers, and other personnel on PT program goals.
• Train patients in use of prosthetic devices when indicated.
• Report patients’ responses to treatment to the physician.
