Job summary

Caringhand Home Health Solutions, Inc. is seeking a compassionate Physical Therapist to provide in-home services in California. This per diem, field-based role requires travel to patients’ homes and requires assessing functional status, mobility, and rehabilitation potential.

You will develop therapy programs, educate patients and caregivers, and coordinate with physicians to support patient recovery and independence. Prior home health experience and a CA license are preferred.

Qualifications

• Bachelor's or master's degree in physical therapy from an appropriately accredited program.
• Current California Physical Therapist license issued by the Physical Therapy Board of California.
• At least two years of professional experience as a Physical Therapist.
• Current CPR certification by American Heart Association.

Responsibilities

• Provide physical therapy services according to the physician's plan of care and California requirements.
• Assess and evaluate patients' status, home environment, and rehabilitation potential.
• Assess strength, mobility, gait, ROM, and other functional abilities relevant to treatment.
• Develop and direct therapy programs; educate patients and caregivers.
• Coordinate with physicians and other healthcare professionals to support patient recovery.
