Job summary

All Care and Comcare Home Health is seeking a Field Physical Therapist in California to provide compassionate, in-home physical therapy tailored to each patient. This role emphasizes flexible scheduling and a supportive team environment.

You will perform SOC/ROC/Recert/DC evaluations, collaborate with the care team, educate families, and travel to patient homes for assessments and treatments. A valid California PT license and CPR certification are required, along with a driver's license and

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.

Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• SOC, ROC, Recert, and DC evaluations.
• Assess patient needs, plan interventions, evaluate outcomes, and modify care.
• Collaborate with the interdisciplinary team to develop patient goals.
• Educate and train patients, families, and caregivers on therapy programs and home exercises.
