Job summary

Caringhand Home Health Solutions, Inc. is seeking a compassionate Physical Therapist to provide in-home physical therapy services across California. You will assess functional status, mobility, gait, and ROM, and develop patient-centered treatment plans with caregivers and physicians.

The role requires independent work, strong clinical assessment, and effective communication. Travel to patient homes is required; this is a field-based, per diem position, not telehealth.

Qualifications

• Strong verbal and written communication skills.
• Ability to work independently and coordinate care in home environments.
• Good organizational skills and the ability to manage patient care responsibilities effectively.
• Previous community-based or home health physical therapy experience.

Responsibilities

• Provide physical therapy services according to the physician's plan of care and California PT Practice Act requirements.
• Assess patients' functional status, mobility, gait, range of motion, and rehabilitation potential.
• Develop and direct appropriate therapy programs and educate patients and caregivers.
• Coordinate with physicians and other healthcare professionals to support patient recovery and independence.
• Supervise PT Assistants as required and educate staff as needed.
• Document clinical progress and participate in discharge planning.
