All Care and Comcare Home Health in California is seeking a compassionate Field Physical Therapist to provide in-home care, delivering skilled PT services with a flexible schedule and supportive team. You will perform SOC/ROC/Recert/DC evaluations, develop individualized goals with the interdisciplinary team, and educate patients and families on home exercise programs.

Requirements include California PT license, CPR, driver's license, auto insurance, and willingness to travel within the service

Qualifications

• Active California Physical Therapy license.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexible regarding work locations across service area.

Responsabilités

• Provide comprehensive, skilled physical therapy services per physician orders.
• Perform SOC, ROC, Recert, and DC evaluations.
• Assess patient needs, plan interventions, evaluate outcomes, and adjust care to meet goals.
• Collaborate with interdisciplinary team to develop and evaluate individualized goals.
• Educate, instruct, and train patients, families, and caregivers on PT programs and home exercises.
