Job summary

Passion-Care in New Jersey is seeking a compassionate Registered Nurse (RN) to perform periodic check-ups for home care patients. You will conduct routine health assessments, monitor chronic conditions, and educate patients and families to support ongoing wellness.

Responsibilities include wellness visits to patients' homes, evaluating vital signs, updating care plans, and coordinating with physicians and care teams.

Qualifications

• Current and valid RN license in NJ.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Reliable transportation and valid driver’s license.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct routine wellness check-ups and health assessments at patients' homes.
• Monitor and evaluate patient condition and vital signs.
• Review and update care plans based on clinical findings.
• Provide education and support to patients and families on managing health conditions.
• Collaborate with physicians, home health aides, and care coordinators.
• Maintain accurate documentation and timely reporting of visits.
• Identify and report any changes in patient condition that require medical attention.
• Ensure compliance with healthcare regulations and safety standards.
