Job summary

Passion Care is seeking a compassionate Registered Nurse (RN) to perform periodic check-ups on patients receiving care in their homes in New Jersey. The RN will conduct routine health assessments, monitor chronic conditions, educate patients and caregivers, and collaborate with the care team to ensure high-quality, person-centered care.

Responsibilities include wellness check-ups, vital sign monitoring, updating care plans, and maintaining documentation.

Qualifications

• Current and valid RN license in NJ.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Reliable transportation and valid driver’s license.
