Job summary

Passion Care in New Jersey is seeking a compassionate Registered Nurse (RN) to perform periodic home health visits. You will conduct routine health assessments, monitor chronic conditions, educate patients and caregivers, and work closely with the care team to deliver high-quality, person-centered care.

The role requires an active NJ RN license, strong clinical assessment and communication skills, reliable transportation, and the ability to work independently.

Qualifications

• Current and valid NJ RN license.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Reliable transportation and valid driver’s license.
• Ability to work independently and manage time effectively.
• Familiarity with electronic medical records (EMR) systems preferred.

Responsibilities

• Conduct routine wellness check-ups and health assessments at patients' homes.
• Monitor and evaluate patient condition and vital signs.
• Review and update care plans based on clinical findings.
• Provide education and support to patients and families on managing health conditions.
• Collaborate with physicians, home health aides, and care coordinators.
• Maintain accurate documentation and timely reporting of visits.
• Identify and report any changes in patient condition that require medical attention.
• Ensure compliance with healthcare regulations and safety standards.
