Qualifications

• Must hold MA State Physical Therapist license.
• Ability to work in a dynamic, cooperative team.
• Capable of evaluating and teaching patients.
• Collaborate with residents, staff, families, and professionals.
• Ability to discuss and plan discharge plans.
• Teach and counsel patients and staff on best practices.

Responsibilities

• Provide physical therapy services with quality rehabilitation care.
• Evaluate patients in a timely manner per requirements.
• Screen patients on admission or when requested.
• Develop a plan of treatment and goals for each patient.
• Provide treatment according to the planned plan after evaluation.
• Communicate with other disciplines and staff as needed.
• Document treatment charges per Medicare and payer guidelines.
• Participate in family conferences when requested.
• Submit required documentation and charges timely.
