Qualifications

• Current MA State License as a Physical Therapist.
• Ability to work as part of a dynamic and cooperative team while ensuring workload is distributed equitably across the department.
• Capability to evaluate and assess patients and participate in consultations and teaching.
• Ability to collaborate with residents, staff, families, and other professionals to plan individualized treatment plans and goals.
• Ability to discuss and plan appropriate discharge plans.
• Ability to teach and counsel patients and staff on best practices for recovery.

Responsibilities

• Provide physical therapy services to the facility, promoting excellence in rehabilitation care, interdisciplinary cooperation, and quality patient care.
• Evaluate patients in a timely manner, as required by state, payer source, or facility requirements.
• Screen patients upon admission and/or when requested by the director.
• Develop an appropriate plan of treatment and goals for each patient.
• Provide treatment according to the plan developed after patient evaluation.
• Communicate with other disciplines and interact with staff as necessary based on the treatment plan.
• Provide routine documentation according to Medicare, facility, and other payer source guidelines.
• Participate in family conferences when requested.
• Submit all required documentation and treatment charges in a timely manner, in accordance with policy and procedure.
