Provide physical therapy services that promote excellence in rehabilitation care, interdisciplinary cooperation, and quality patient outcomes.
Evaluate and assess patients in a timely manner according to state, payer, and facility requirements.
Develop individualized treatment plans and goals based on each patient's needs and provide treatment accordingly.
Provide consultations and collaborate with other healthcare professionals to promote optimal patient outcomes.
Communicate effectively with staff, patients, families, and other disciplines to ensure coordinated care.
Document treatment plans, goals, interventions, and patient progress according to Medicare and other payer guidelines.
Participate in family conferences and provide updates regarding patient progress as needed.
Complete and submit timely documentation and treatment charges in accordance with facility policies and procedures.
Promote patient safety, independence, dignity, and quality of life throughout the rehabilitation process.
Current Massachusetts state license as a Physical Therapist.
Ability to work effectively as part of a dynamic and cooperative interdisciplinary team.
Strong evaluation, assessment, clinical reasoning, and treatment-planning skills.
Ability to develop individualized treatment plans based on patient needs and goals.
Strong communication and interpersonal skills.
Ability to educate, teach, and counsel patients, families, and staff regarding the rehabilitation process.
