Job summary

LTC Therapy Services in Worcester, MA is seeking a licensed Physical Therapist (PT) to join our rehabilitation team in a full-time capacity. You will evaluate residents, develop individualized plans of care, and guide mobility and functional recovery in a skilled nursing and post-acute setting.

You'll provide skilled treatment focusing on strength, balance, gait, and safety, set measurable goals, document accurately, and collaborate with PTAs, OT, SLP, nursing, and other team members.

Qualifications

• Active Massachusetts Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• New graduates welcome to apply.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and functional mobility.
• Establish measurable rehabilitation goals and monitor resident progress.
• Adjust treatment approaches based on clinical findings and response to therapy.
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies.
• Complete accurate and timely clinical documentation.
• Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary team.
