Stellar Health Group's Woburn Rehabilitation and Nursing Center is seeking a compassionate Physical Therapist to join our therapy team. We offer Full-Time, Part-Time, and PRN opportunities to help patients improve mobility and regain independence. A $7,500 sign-on bonus is available.

You will evaluate, treat, and develop individualized treatment plans, collaborate with an interdisciplinary team, and document per payer guidelines. Massachusetts PT license is required.

Qualifications

• Massachusetts state license as a Physical Therapist is required.
• Ability to work in a dynamic, cooperative interdisciplinary team.
• Strong evaluation, assessment, and treatment planning skills.
• Capability to develop individualized treatment plans based on patient needs.
• Excellent communication and interpersonal skills.
• Ability to educate patients, families, and staff on rehabilitation.

Job Summary

Woburn Rehabilitation and Nursing Center, part of Stellar Health Group, is seeking a compassionate and dedicated Physical Therapist (PT) to join our energetic therapy team. We offer Full-Time, Part-Time, and PRN opportunities for professionals who are passionate about helping patients improve their mobility, regain independence, and achieve their rehabilitation goals.

Responsibilities

• Provide physical therapy services that promote excellence in rehabilitation care, interdisciplinary cooperation, and quality patient outcomes.
• Evaluate and assess patients in a timely manner according to state, payer, and facility requirements.
• Develop individualized treatment plans and goals based on each patient's needs and provide treatment accordingly.
• Provide consultations and collaborate with other healthcare professionals to promote optimal patient outcomes.
• Communicate effectively with staff, patients, families, and other disciplines to ensure coordinated care.
• Document treatment plans, goals, interventions, and patient progress according to Medicare and other payer guidelines.
• Participate in family conferences and provide updates regarding patient progress as needed.
• Complete and submit timely documentation and treatment charges in accordance with facility policies and procedures.
• Promote patient safety, independence, dignity, and quality of life throughout the rehabilitation process.

Qualifications

• Current Massachusetts state license as a Physical Therapist.
• Ability to work effectively as part of a dynamic and cooperative interdisciplinary team.
• Strong evaluation, assessment, clinical reasoning, and treatment-planning skills.
• Ability to develop individualized treatment plans based on patient needs and goals.
