Job summary

Therapy Management Corporation (TMC) is seeking a Physical Therapist to empower adults and older adults through compassionate, patient-centered care. You will evaluate and treat an adult/geriatric population with an emphasis on independence and mobility in a collaborative clinical setting.

Qualifications

• Licensed PT or license eligible in the state of practice.
• Graduate of an accredited PT program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments that help patients improve mobility, strength, balance, and functional independence
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals
• Deliver hands‑on therapy interventions designed to support recovery, increased endurance, and enhance overall quality of life in older adults
• Help patients restore functional mobility and safely perform daily activities with greater confidence and independence
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes and continuity of care
• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals
