Qualifications

• Licensed Physical Therapist (PT) in practicing state or eligible for licensure
• Experience or interest in skilled nursing, long-term care, or geriatric rehab
• Passion for improving quality of life for older adults and delivering patient-centered care
• Ability to work in a collaborative clinical environment with autonomy and clinical judgment

Responsibilities

• Evaluate patients and provide personalized PT treatments to improve mobility, strength, balance, and independence
• Develop and implement patient-centered treatment plans for long-term rehab goals
• Deliver hands-on interventions to support recovery and daily function in older adults
• Help patients restore functional mobility and perform daily activities with confidence and safety
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive outcomes and continuity of care
• Track progress, document accurately, and adapt plans based on rehab goals
