Qualifications

• Must be licensed PT or eligible for licensure in the state of practice.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance, and independence.
• Develop and implement patient-centered treatment plans for long-term rehabilitation goals.
• Deliver hands-on therapy to support recovery and endurance in older adults.
• Help patients restore functional mobility for daily activities with greater confidence and independence.
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes and continuity of care.
• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans as needed.
