Qualifications

• Licensed Physical Therapist (PT) or license eligible in the state of practice
• Graduate of an accredited Physical Therapist Program
• Interest or experience in skilled nursing, long‑term care or geriatric rehabilitation
• Passion for patient-centered care and improving quality of life for older adults

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance, and independence
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals
• Deliver hands-on therapy interventions to support recovery and enhance quality of life in older adults
• Help patients restore functional mobility and perform daily activities with confidence and independence
• Collaborate with interdisciplinary teams, caregivers, and nursing staff to drive meaningful outcomes and continuity of care
• Track progress and adapt treatment plans based on patient needs and rehabilitation goals
