Job summary

Telos Health Systems seeks a Psychotherapist (LCSW, LCMHC, LMFT, or Psychologist – PsyD/Ph.D.) to deliver assessments and psychotherapy in skilled nursing facilities and assisted living facilities across NC. The role offers both part-time and full-time options with strong administrative and leadership support.

You will conduct evaluations, develop treatment plans with a multidisciplinary team, and ensure compliant documentation while coordinating care with facility staff and medical providers.

Qualifications

• Licensed in one or more of the following: LCSW, LCMHC, LMFT, PsyD/Ph.D.
• Active, unrestricted license in the practicing state.
• Ability to work independently while collaborating with a clinical team.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop and update individualized treatment plans with a multidisciplinary team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff and other healthcare professionals.
• Adhere to federal, state, and organizational standards, policies, and regulations.
