A Path of Care Home Health in Yukon, OK is seeking a PRN Speech Therapist to join our home health team. The role focuses on evaluating and treating patients in their homes, supporting physicians, and documenting care plans in line with CMS requirements.

You will help develop plans of care, prepare clinical notes, and communicate with families and agency staff. The position offers flexible PRN hours and the chance to make a direct impact on patients’ lives.

Qualifications

• One year of speech therapy experience.
• Current license in good standing with the state of Oklahoma medical board.
• CPR certification or the ability to obtain one within 60 days of being hired.
• Ability to meet the physical demands of the job.

Responsabilités

• Assess patients and contribute to plan of care.
• Develop or revise plans of care and prepare progress notes.
• Consult with family and other agency personnel.
• Provide services per CMS documentation and billing requirements.
• Maintain infection control and follow universal precautions.
• Support orientation for new hires and participate in training.
