Job summary

Excell Home Care & Hospice seeks a compassionate PRN Speech-Language Pathologist to support home health patients in Oklahoma City. You will perform evaluations, develop care plans, and provide one-on-one therapy to improve communication, cognition, and swallowing, while educating patients and families.

The role requires a master’s or doctoral degree in SLP, Oklahoma licensure, CPR certification, and a valid driver’s license.

Qualifications

• Must have a master’s or doctoral degree in speech-language pathology and be licensed in the state where practicing.
• If licensure is not required, must have completed 350 clock hours of supervised practicum.
• Minimum of one year’s clinical experience in Speech–Language Pathology.

Responsibilities

• Conduct evaluations and assess home environment as appropriate.
• Develop and update individualized plan of care in partnership with patient and caregiver.
• Provide services ordered by physician and educate patient, caregiver and family.
• Prepare clinical notes and communicate with the physician and other health practitioners.
• Identify functional speech defects and establish a plan to improve function.
• Administer speech therapy program using specialized techniques and equipment.
• Supervise home health aide when related to speech-language care.
• Participate in quality assessment and staff training.
