Job summary

Stern Therapy Consultants in Ayer, MA seeks a Physical Therapist (PT) on a PRN basis to provide individualized care focused on improving strength, mobility, balance, and functional independence.

You will perform evaluations, establish plans of care, and coordinate with the inter-disciplinary team to support residents’ rehabilitation goals. New graduates are encouraged to apply for this flexible opportunity.

Qualifications

• Active Massachusetts Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical judgment, communication, and teamwork skills.
• Skilled nursing or post-acute rehabilitation experience preferred, but not required.
• New graduate Physical Therapists are encouraged to apply.

Responsibilities

• Complete physical therapy evaluations and establish individualized plans of care.
• Provide skilled interventions for gait, strength, balance, transfers, endurance, and mobility.
• Develop appropriate rehabilitation goals and track resident progress.
• Modify treatment plans based on clinical findings and response to therapy.
• Educate residents and caregivers on therapeutic exercises and safe mobility techniques.
• Maintain clear and timely clinical documentation.
• Coordinate care with PTAs and other members of the interdisciplinary team.
