Job summary

Stern Therapy Consultants in Wolfeboro, NH is seeking a Physical Therapist (PT) to provide PRN rehabilitation services at Wolfeboro Bay Center. The PT will evaluate residents and develop individualized plans of care to improve movement, strength, balance, and overall mobility.

The role offers a flexible PRN schedule in a supportive rehabilitation setting with opportunities to work with a diverse resident population across short-term and long-term care.

Qualifications

• Active New Hampshire PT license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and organizational skills.
• Skilled nursing or post-acute rehabilitation experience preferred, but not required.
• New graduate Physical Therapists are welcome to apply.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide skilled interventions addressing strength, balance, gait, transfers, endurance, and mobility.
• Establish measurable rehabilitation goals and track resident progress.
• Adjust treatment plans based on clinical findings and response to therapy.
• Educate residents and caregivers on exercises and safe mobility strategies.
• Complete required clinical documentation accurately and on time.
• Collaborate with PTAs, OT, SLP, nursing, and other members of the care team.
