Job summary

Stern-Therapy-Consultants in Agawam is seeking a PRN Physical Therapist (PT) to join their rehabilitation team at a skilled nursing facility. The role offers flexible scheduling and opportunities to work with residents aiming to improve mobility, balance, strength, and overall functional independence.

Applicants must hold an active Physical Therapist license and possess strong communication skills. Experience in skilled nursing is preferred but not required. This role allows for a collaborative environment with interdisciplinary teamwork.

Responsibilities

• Complete physical therapy evaluations and provide individualized treatment sessions.
• Address gait training, transfers, balance, strengthening, endurance, and mobility goals.
• Collaborate with rehab staff, nursing, and physicians to support resident care plans.
• Monitor resident progress and adjust treatment plans as needed.
• Maintain timely and accurate documentation.
