Job summary

Stern-Therapy-Consultants in Agawam is seeking a PRN Physical Therapist (PT) to join their rehabilitation team at a skilled nursing facility. Responsibilities

• Complete physical therapy evaluations and provide individualized treatment sessions.
• Address gait training, transfers, balance, strengthening, endurance, and mobility goals.
• Collaborate with rehab staff, nursing, and physicians to support resident care plans.
• Monitor resident progress and adjust treatment plans as needed.
• Maintain timely and accurate documentation.
