Job summary

Stern Therapy Consultants in Worcester, MA is seeking a Physical Therapist (PT) for a PRN position. You will evaluate residents, develop plans of care, and provide interventions to improve gait, balance, transfers, endurance, and mobility in a skilled nursing or post-acute setting.

The role requires an active MA PT license, a degree from an accredited PT program, and strong clinical and communication skills. Experience in skilled nursing or post-acute rehab is preferred but not required.

Qualifications

• Active Massachusetts Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and organizational skills.
• Skilled nursing or post-acute rehabilitation experience preferred, but not required.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide skilled interventions addressing gait, strength, balance, transfers, endurance, and mobility.
• Establish appropriate rehabilitation goals and monitor resident progress.
• Modify treatment plans according to clinical findings and response to therapy.
• Educate residents and caregivers on therapeutic exercises and safe mobility strategies.
• Maintain clear and timely clinical documentation.
• Coordinate care with PTAs and other members of the interdisciplinary team.
