Job summary

Silver Memories Health Care in Versailles, IN is seeking a PRN Physical Therapist to join our skilled nursing team. This role offers hands-on therapy to residents aiming to regain strength, mobility, and independence with a flexible part-time schedule.

You will evaluate residents, develop individualized treatment plans, implement interventions to improve balance, endurance, and functional mobility, and communicate progress with residents and families.

Qualifications

• Doctorate, Master’s, or Bachelor’s degree from an accredited Physical Therapy program.
• Active Indiana PT license.
• Commitment to ethical, patient-centered care.
• Strong communication and interpersonal skills.
• Willingness to participate in ongoing professional development.

Responsibilities

• Provide physical therapy services and develop individualized treatment plans based on residents’ needs and functional goals.
• Evaluate residents and establish appropriate treatment plans in accordance with professional standards of practice.
• Implement therapeutic interventions focused on improving strength, mobility, balance, endurance, and functional independence.
• Monitor resident progress and modify treatment approaches as appropriate.
• Document resident progress accurately and communicate effectively with residents, families, and the interdisciplinary care team.
