Job summary

Woodland Manor in Elkhart, IN seeks a compassionate Physical Therapist (PT) to join our rehabilitation team on a PRN basis. You will evaluate residents, develop individualized plans, and help improve mobility, balance, and independence in a supportive environment.

As part of an interdisciplinary team, you’ll monitor progress, adjust goals, educate residents and families, and maintain accurate documentation in compliance with standards, delivering patient-centered care with empathy.

Qualifications

• Indiana license to practice physical therapy is required.
• BLS certification is current.
• Strong clinical judgment and communication skills.
• Compassionate, patient-centered approach to care.
• Ability to work independently while collaborating with rehab teams.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide evidence-based, patient-centered therapy interventions to improve mobility, strength, balance, and functional independence.
• Monitor resident progress, update goals, and modify treatment plans as appropriate.
• Collaborate with the interdisciplinary care team to support coordinated and effective rehabilitation outcomes.
• Maintain accurate documentation and ensure compliance with clinical and regulatory standards.
• Educate residents, families, and caregivers on exercises and strategies that support continued progress and independence.
