Job summary

Woodland Manor Nursing and Rehab in Elkhart, IN is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team on a PRN, as-needed basis. You will help residents improve strength, mobility, balance, and independence while delivering patient-centered care in a supportive environment.

Qualifications

• Licensed Physical Therapist (PT) in the state of Indiana
• Current BLS certification
• Strong clinical judgment and communication skills
• Compassionate, patient-focused approach to care
• Ability to work independently while collaborating with rehab teams

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Provide evidence-based, patient-centered therapy interventions to improve mobility, strength, balance, and functional independence
• Monitor resident progress, update goals, and modify treatment plans as appropriate
• Collaborate with the interdisciplinary care team to support coordinated and effective rehabilitation outcomes
• Maintain accurate documentation and ensure compliance with clinical and regulatory standards
• Educate residents, families, and caregivers on exercises and strategies that support continued progress and independence
