Job summary

Simpsonville Post Acute in Simpsonville, SC is seeking a licensed Physical Therapist (PT) to provide high-quality, patient-centered therapy on a PRN basis. You will evaluate residents, develop treatment plans, document care, and work as part of an interdisciplinary team.

Responsibilities include supervising PTAs, educating residents and families, participating in care planning, and ensuring compliance with regulations. A SC license and experience in rehab are required or preferred.

Qualifications

• Licensed Physical Therapist in South Carolina in good standing.
• Experience in skilled nursing or rehabilitation setting preferred.
• Familiarity with PointClickCare and Casamba preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide skilled therapy services per physician orders and care plans
• Document evaluations, treatments, and progress notes
• Communicate effectively with physicians, nursing staff, and care team
• Supervise PTAs, aides, and students per state acts
• Participate in care planning and interdisciplinary meetings
• Educate residents, families, and caregivers on therapy goals and discharge plans
• Maintain compliance with state, federal, and payer guidelines
