Job summary

Simpsonville Post Acute in Simpsonville, SC is seeking a licensed Physical Therapist to provide patient-centered therapy on a PRN basis within an interdisciplinary team. The role emphasizes mobility, independence, and quality care for residents.

Qualifications

• Licensed Physical Therapist with SC license in good standing.
• Bachelor’s degree in Physical Therapy; advanced degree preferred.
• Experience in skilled nursing or rehabilitation setting preferred.
• Familiarity with PointClickCare and Casamba preferred.
• Strong clinical, documentation, and communication skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide therapy services per physician orders and care plans.
• Document evaluations, treatments, and progress notes.
• Communicate effectively with physicians, nursing staff, and care team members.
• Supervise PTAs, aides, and students in accordance with state practice acts.
• Participate in care planning and interdisciplinary meetings.
• Educate residents, families, and caregivers on therapy goals and discharge plans.
• Conduct staff training and in-service education.
• Maintain compliance with all state, federal, and payer guidelines.
