Job summary

Simpsonville Post Acute in Simpsonville, SC is seeking a licensed Physical Therapist Assistant (PTA) to provide high-quality, patient-centered therapy under a Physical Therapist's supervision. You will help residents improve mobility and independence, document treatments and progress, follow physician plans, and participate in care planning with the interdisciplinary team.

Requirements include an active SC PTA license, graduation from an accredited PTA program, and preferred experience in

Qualifications

• Active PTA license in SC (in good standing).
• Graduate of an accredited PTA program.
• Experience in skilled nursing or rehab setting preferred.
• Familiarity with PCC and Casamba preferred.
• Strong clinical, documentation, and communication skills.

Responsibilities

• Provide therapy treatments as directed by the Physical Therapist.
• Follow physician treatment plans and care goals.
• Document daily treatments and weekly progress notes accurately.
• Assist with rehabilitation and care planning meetings.
• Communicate patient progress with the interdisciplinary team.
• Assist with discharge planning and caregiver education.
• Maintain clean, safe therapy areas and equipment.
• Support training of restorative aides and nursing staff.
• Ensure compliance with state practice act and facility policies.
