Job summary

Simpsonville Post Acute is seeking a licensed Physical Therapist Assistant (PTA) to provide high-quality, patient-centered therapy under the supervision of a Physical Therapist in Simpsonville, SC. You will help residents improve mobility, strength, and independence while following established treatment plans.

Responsibilities include documenting treatments, communicating progress to the interdisciplinary team, and supporting discharge planning.

Qualifications

• Active Physical Therapist Assistant license in good standing in South Carolina.
• Graduate of an accredited PTA program.
• Experience in skilled nursing or rehab setting preferred.
• Familiarity with PointClickCare (PCC) and Casamba preferred.
• Strong clinical, documentation, and communication skills.

Responsibilities

• Provide therapy treatments as directed by the Physical Therapist.
• Follow physician treatment plans and care goals.
• Document daily treatments and weekly progress notes accurately.
• Assist with rehabilitation and care planning meetings.
• Communicate patient progress with the interdisciplinary team.
• Assist with discharge planning and caregiver education.
• Maintain clean, safe therapy areas and equipment.
• Support training of restorative aides and nursing staff.
• Ensure compliance with state practice act and facility policies.
