Job summary

Afton Oaks Nursing & Rehabilitation Center in Houston, TX seeks a dedicated Physical Therapist (PT) to join our in-house team. You will help residents improve mobility, strength, and overall quality of life through person-centered care and a collaborative approach with the interdisciplinary team.

The ideal candidate holds a Texas PT license, has SNF or long-term care experience, and is skilled in diverse PT methods. In-house rehab offers flexibility and supportive corporate backing.

Qualifications

• Active and valid Texas Physical Therapist license.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health EDS preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Monitor patient response to treatment interventions and adjust approaches as appropriate with the interdisciplinary team.
• Complete documentation thoroughly and timely in compliance with policies and state/federal regulations.
• Attend required meetings as designated by the Director of Rehab.
