Job summary

Fortress Nursing and Rehabilitation in Texas is seeking a dedicated Physical Therapist (PT) to join our in-house team. You will help residents improve mobility and quality of life through personalized treatment plans.

Responsibilities include assessments, developing rehab programs, administering PT treatments, and collaborating with the care team. SNF experience and active TX PT license are preferred; Net Health experience is a plus. In-house rehab and flexible schedules offered.

Qualifications

• Active and valid Texas Physical Therapist license required.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain and restore function.
• Ensure treatments align with the established plan of care and state/federal regulations.
• Monitor patient response and modify approaches in collaboration with the interdisciplinary team.
• Provide education to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Complete documentation thoroughly and timely.
