Job summary

Christian Living Communities offers a flexible, as-needed Rehabilitation Flex Team position providing skilled nursing, assisted living, and rehabilitation services. The role focuses on building trust with residents while administering medically prescribed physical therapy to improve mobility and independence.

Rehabilitation team members work across sites, with a strong emphasis on resident-centered care, communication, and collaboration with physicians and staff.

Qualifications

• Graduate of an accredited therapy program.
• 2–3 years of related experience or equivalent combination.
• Current Physical Therapist state license; CPR certification.
• Excellent verbal & written communication, active listening and social perceptiveness.
• Relationship building, compassionate instruction and appreciation for individuality.

Responsibilities

• Restores residents’ function and alleviates pain through medically prescribed physical therapy.
• Provides quality, personalized care by assessing evaluations and test results.
• Develops a Plan of Treatment with goals and collaboration with staff and residents.
• Implements the Plan of Treatment on a schedule and adjusts as needed.
• Communicates progress to residents, caregivers and healthcare professionals.
• Develops transition plans for home-based services and coordinate care.
• Collaborates with team to foster a diverse, supportive workplace culture.
