Job summary

Christian Living Communities | Cappella Living Solutions (CLC/CLS) is seeking a Rehabilitation Flex Team Physical Therapist to provide skilled nursing, assisted living and rehabilitation services on a flexible, as-needed basis across our communities. The role emphasizes building trust with residents and coordinating with the care team.

The position requires an active Physical Therapist license, CPR certification, and 2–3 years of related experience.

Qualifications

• Graduate of an accredited therapy program.
• 2–3 years of related experience or equivalent.
• Current Physical Therapist state license; CPR certification.
• Excellent verbal & written communication, active listening, and social perceptiveness.
• Relationship building, compassionate instruction, and appreciation for individuality.

Responsibilities

• Restores residents’ function and relieves pain by planning and administering medically prescribed physical therapy.
• Provides quality, personalized care by assessing evaluations and test results.
• Develops a Plan of Treatment with goals and collaboration with staff and residents.
• Determines treatment plans in consultation with physicians or by prescription.
• Implements the Plan of Treatment on a schedule and adjusts as needed.
• Communicates progress with residents, families, and care teams.
