Job summary

The Physical Therapist in Home Health with UnitedHealth Group will perform assessments and evaluations related to mobility, endurance, pain and other conditions, and help determine a treatment plan to improve well-being and progress.

Responsibilities include delivering care within the plan, educating patients and families, documenting visits, and reporting outcomes. This per diem role offers flexible scheduling and a supportive team environment.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and PT needs and assists with therapy plan of care.
• Provides services within scope of practice and plan of care in coordination with the health care team.
• Observes and reports patient response and changes to supervising staff or physician.
• Treats patients to relieve pain, restore function and maintain maximum functioning.
• Instructs patient, family and care team regarding plan of care and progress.
• Visits patients per Plan of Treatment, documents progress notes per policy.
• Completes evaluations and develops PT plan within state guidelines.
• Reports outcomes and anticipated frequency of care.
• Function effectively in diverse home settings.
