Job summary

Elara Caring in Mishawaka, IN seeks a Private Duty Registered Nurse Case Manager for part-time, 5-hour shifts, Mon-Fri. You will assess patient needs, develop individualized care plans, and coordinate with the care team to ensure seamless home-based services. A valid RN license and 2+ years in clinical/home health are preferred.

You’ll report to the Branch Director or Clinical Manager and join a supportive team delivering compassionate, patient-centered care in patients’ homes.

Qualifications

• Graduate of an accredited RN Program.
• 2+ years’ experience in a clinical care setting or home health environment preferred.
• Current state license as RN.
• Excellent communication skills.
• Dedicatio n to patient care.
• Ability to work as part of inter-disciplinary team.

Responsibilities

• Assesses health needs to determine home care admission and develops a personalized plan of care.
• Coordinates with clients, caregivers, physicians to ensure continuity of care and timely transitions.
• Implements and updates the plan with caregivers and supervisors, monitoring progress.
• Supervises Private Duty RN, LPN and Home Health Aides to ensure quality of care.
• Reviews documentation to ensure alignment with the plan of care.
