Job summary

Center4Hospice in Mishawaka, IN seeks an experienced Registered Nurse/Case Manager to provide comprehensive home-based hospice care. You will assess patients, develop care plans, manage pain, and collaborate with the interdisciplinary team to enhance quality of life.

This full-time role requires an Indiana RN license, current CPR, and a valid driver’s license. You'll visit patients at their homes, document care, and participate in IDG meetings, with a supportive benefits package and ongoing

Qualifications

• Licensed RN in Indiana with current CPR certification.
• Must hold a valid driver’s license and auto insurance.
• Ability to pass drug screen and background check; TB test.

Responsibilities

• Performs an initial and ongoing comprehensive assessment within 5 days of hospice election.
• Develops an individualized plan of care with patient/family goals.
• Assesses pain and comfort; implements a pain management plan.
• Provides holistic, patient/family-centered care at the patient’s residence.
• Collaborates with the interdisciplinary team and other agencies.
• Supports patient/family in medical decisions and spiritual beliefs.
• Supervises LPN and Home Health/Hospice Aides per policy.
• Performs admissions and completes about 25 visits per week.
• Documents assessments, goals, care provided, and outcomes.
• Completes day-of-care patient assessment documentation.
