Qualifications

• Active RN license in the state of practice.
• Experience in home health, skilled nursing, or related setting preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct Start of Care (SOC) visits to complete comprehensive assessments and develop physician-directed care plans.
• Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans.
• Provide hands-on skilled nursing including wound care, medication management, injections, and chronic disease management.
• Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations.
• Collaborate with physicians and interdisciplinary team to ensure coordinated, high-quality care.
• Maintain accurate and timely clinical documentation in accordance with state regulations and agency standards.
• Supervise and delegate care to LPNs and Home Health Aides to ensure plan adherence.
