Elara Caring is seeking a Private Duty Registered Nurse Case Manager for a part-time role in Mishawaka, IN and surrounding areas. You will assess patients at home, develop personalized care plans, and coordinate with physicians, caregivers, and home health staff to ensure safe, timely, and quality care.

The role requires an accredited RN program graduate with 2+ years of clinical or home health experience, an active RN license, and strong communication and teamwork skills.

Qualifications

Responsibilities
