Job summary

Elara Caring is seeking a Private Duty Registered Nurse Case Manager for a part-time role in Mishawaka, IN and surrounding areas. You will assess patients at home, develop personalized care plans, and coordinate with physicians, caregivers, and home health staff to ensure safe, timely, and quality care.

The role requires an accredited RN program graduate with 2+ years of clinical or home health experience, an active RN license, and strong communication and teamwork skills.

Qualifications

• Graduate of an accredited RN program.
• 2+ years’ experience in a clinical or home health setting.
• Current state RN license.
• Excellent communication skills.
• Ability to work as part of an inter-disciplinary team.

Responsibilities

• Assess health needs and determine home care admission.
• Develop and coordinate plans of care with clinicians and caregivers.
• Coordinate services (equipment, pharmacy, transportation) to ensure continuity.
• Provide supervision of RNs, LPNs, and HHAs to ensure quality.
• Review documentation for adherence to plan of care.
