Job summary

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Active RN license in Indiana.
• Minimum 1 year of acute care or home health experience.
• Current CPR certification.

Responsibilities

• Conduct initial assessments and ongoing evaluations of symptoms and care needs.
• Develop, implement, and monitor individualized care plans.
• Complete and submit OASIS documentation in a timely manner.
• Collaborate with physicians, therapists, social workers and other professionals.
• Supervise HHAs when appropriate and coordinate care teams.
• Deliver skilled nursing services in the home, including wound care and IV therapy.
• Maintain accurate, detailed, and timely patient care documentation.
