Baywood Home Care is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate care, support patients and families, and help individuals maintain their independence in the comfort of their own homes.

This part-time (up to 20 hours/week), in-person role offers flexible scheduling, competitive pay between $37.44 and $45.00 per hour, mileage reimbursement, and opportunities for on-call shifts.

Qualifications

• Current Minnesota RN license required.
• Home health, case management, or transitional care experience required.
• Strong assessment, communication, and care coordination skills.
• At least 21 years old.

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Coordinate services with physicians, therapists, caregivers, and community resources.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and families on medications, treatment plans, and wellness strategies.
• Maintain accurate clinical documentation and regulatory compliance.
