Youth Advocate Programs, Inc. seeks a Family Support Specialist in Mount Union, PA.

This part-time hourly role supports families through community-based services, with a non-exempt status and flexible hours up to 40 per week, including potential non-traditional shifts. You will develop service plans, coordinate with schools, hospitals, and social agencies, provide counseling and advocacy, and document progress weekly.

Qualifications

• Bachelor’s degree in mental health related fields preferred but not required.
• One to two years experience in community-based family work.
• Proficiency with data collection tools; knowledge of EHR systems a plus.
• Proficient with Microsoft Office; reliable transportation and valid driver's license.
• Bilingual Spanish speaking is a plus.

Responsibilities

• Deliver quality community-based services to families.
• Develop individualized service plans with goals from referrals.
• Coordinate with social agencies, hospitals, schools and resources.
• Provide counseling, advocacy, and service coordination with families.
• Prepare monthly meetings, assessments, and daily progress notes.
• Submit incident reports and communicate with supervisors.
