Job summary

Youth Advocate Programs, Inc. is seeking a Family Support Specialist in Pennsylvania.

This part-time hourly role supports community-based services, with a flexible schedule and up to 40 hours weekly. You will develop individualized service plans, coordinate with social agencies, hospitals, schools and community resources, provide counseling, advocacy and care coordination to families, and document progress and incidents.

Qualifications

• Bachelor's Degree in Mental Health related fields preferred, not required.
• One to two years related experience in community-based family work.
• Proficiency in using data collection tools and software applications. Knowledge of using an Electronic Health Record (EHR) System is a plus.
• Proficient using Microsoft Office (O365) Suite.
• Strong organizational and communication skills.
• Reliable transportation, valid driver's license, and current auto insurance coverage.
• Bilingual (Spanish Speaking) is a plus.

Responsibilities

• Develop individualized service plans incorporating identified goals.
• Assist with coordination and implementation of community-based services with social agencies, hospitals, clinics, schools and community resources.
• Provide counseling, support, advocacy and service coordination with families both individually and as part of a team.
• Report special, unusual or critical incident reports and update supervisors on participant activities.
• Assist with monthly meetings for all participants and teams.
• Complete typed and written assessments, closings/aftercare plans and daily progress notes in addition to weekly billing documents.
